Southeastern San Diego Health Access Project

Southeastern San Diego Health Access Project provides equitable access to healthcare in Southeast region of San Diego.  This project will bring valuable health resources to the community through:

• Community outreach in multiple languages
• Health education workshops
• Access to mobile health services (medical, dental, and vision)
• Targeting O'Farrell and the five (5) feeder elementary schools (Bethune, Encanto, Fulton, Nye, and Valencia Park)
• Case management to ensure continuous health care.
